Program Commitment
I commit to actively participating in the Better Lives Program for a minimum of 90 days. I understand this program is designed to support my transition from temporary to permanent employment.
Weekly Check-ins
I agree to participate in weekly check-in meetings with my case manager. These meetings are essential for tracking progress, addressing challenges, and receiving support.
Communication
I will maintain open and honest communication with my case manager and employer. I will promptly report any issues affecting my work performance or personal stability.
Work Performance
I commit to maintaining satisfactory job performance, including attendance, punctuality, and professional conduct. I understand that my performance will be evaluated as part of this program.
Goal Setting
I agree to work with my case manager to set personal and professional goals. I will actively work toward achieving these goals throughout the program.
